What insurance do you accept?
I am an in-network provider for the following insurance plans:First Choice/Kaiser PPO and Lyra. I am also able to work with most insurance plans (including Premera, Cigna, Regence, Kaiser HMO, Aetna, etc) as an out-of-network (OON) provider. Depending on your insurance plan, a portion of your fees may be reimbursed directly to you by your insurance company, sometimes at a comparable rate to what you would pay using in-network insurance. If using out-of-network benefits, I can provide a reimbursement statement for client to submit to their insurance. Please call your insurance plan directly to determine what your OON benefits might be. Some helpful questions to ask your insurance provider:
Please provide full insurance information and your insurance card upon your initial visit to determine eligibility of benefits, and obtain authorization from your insurance provider when necessary prior to your first visit.
What forms of payment do you accept?
I accept cash, check, and credit card (VISA, MC, Discover, HSA/FSA) payments. Please make checks payable to Suie Chang, PsyD, PLLC. Any returned check will be subject to a fee of $35.
For those who are not utilizing insurance, you have the right to request a Good Faith Estimate.
Do you offer a sliding scale?
No, I do not offer sliding scale for services.
What is your cancellation policy?
A minimum of 24 hours notice, prior to your scheduled appointment time is required for cancellations. Late cancellations are billed at the full rate of the service provided ($225 per session). Please note insurance companies do not cover late cancellation fees.
